Key Responsibilities:

• Administer medications, treatments, and wound care as prescribed
• Follow individualized care plans to ensure quality resident outcomes
• Accurately document observations, interventions, and resident status
• Order prescribed medications, supplies, and equipment
• Uphold resident confidentiality and comply with HIPAA regulations

Qualifications:

• Valid LPN license in the State of New Jersey
• Prior experience in skilled nursing or long-term care preferred
