Nurse Practitioner or Physician Assistant

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments for health plan members. In this role, you will review medical histories, conduct physical exams, reconcile medications, and identify care gaps to support better outcomes and reduced costs. You’ll collaborate with physicians, care managers, and payers while documenting thoroughly in EMR systems.
