Registered Nurse - RN Case Manager

What You Will Do Serve as a key member of the Interdisciplinary Team (IDT), providing holistic, patient- and family-centered hospice care. Complete comprehensive nursing assessments, admissions, and ongoing reassessments in accordance with hospice regulations and timelines. Develop, implement, and continuously evaluate individualized plans of care in collaboration with the IDT and attending physicians. Provide skilled nursing care, including symptom management, pain control, wound care, IV therapy, and physical assessments. Educate patients and families on disease progression, medications, pain and symptom management, and routine caregiving tasks. Provide emotional support and compassionate guidance to patients and families throughout the hospice journey. Fulfill on-call responsibilities as assigned and assist with admissions or coverage during staffing needs. What We Are Looking For Currently licensed as a Registered Nurse in the Commonwealth of Virginia. Minimum of two (2) years of nursing experience. Hospice or home health experience preferred. Strong clinical judgment, critical thinking, and decision-making skills. Excellent communication skills with the ability to educate, support, and collaborate effectively. Proficiency with EMR documentation and computer-based systems. Valid driver’s license, reliable transportation, and proof of automobile insurance.
