Travel Physical Therapist - SNF / LTC - $2,316 per week

Centra Healthcare Allied is seeking a Travel Physical Therapist for a contract assignment in Springtown, Texas. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel The Physical Therapist is responsible for assessing referred patients that require rehabilitation services. Duties include providing direct patient care, evaluating medical conditions, functional capabilities, limitations and potential for rehabilitation, establishing and administering a treatment program with specific goals, setting realistic and achievable goals, documenting patient progress, and educating patients and families on appropriate physical therapy methods. Requirements Graduation from an accredited school. Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure. Current CPR certification. Develop individual care plans for each patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient.
