Description

What We're Looking For in a Clinical Team Leader:

  • Compassionate and caring attitude with a genuine concern for patients
  • Strong leadership, problem-solving, and decision-making skills
  • Excellent interpersonal skills to connect with patients, families, and team members
  • Previous supervisory experience required (minimum 2 years)
  • In-depth understanding of hospice care and clinical protocols

Key Responsibilities:

  • Supervise and coordinate the delivery of high-quality patient care on a 24-hour basis
  • Manage personnel actions such as hiring, onboarding, performance reviews, and scheduling
  • Coordinate patient care issues with families, physicians, and healthcare providers
  • Ensure complete, timely, and accurate documentation of patient care in compliance with policies and regulations
  • Lead interdisciplinary group (IDG) meetings and case conferences to align care goals
  • Oversee regulatory compliance, clinical quality improvement, and staff development initiatives
  • Review and implement policies and procedures to maintain agency standards
  • Cover Management Back-Up (MBU) responsibilities as assigned
  • Case manage patients when needed or requested

Requirements:

  • Current RN license in North Carolina
  • 2 years of supervisory experience required
  • 5 years of recent clinical experience in hospice, home health, or equivalent
  • Current CPR certification
  • Knowledge of reimbursement sources, Medicare/Medicaid regulations, licensing laws, and accreditation standards
  • Must meet pre-employment and ongoing state and federal training requirements
