Staff - Registered Nurse (RN) - Hospice - $66K-88K per year

Job Overview Gentiva is seeking a Registered Nurse (RN) Hospice for a full‑time weekday position in Hendersonville, Tennessee. The role is ongoing staff employment within the hospice specialty and requires an RN licensed to practice in the state of employment. The RN will provide skilled, hands‑on nursing care, perform assessments, collaborate with an interdisciplinary team, and support patients, families, and caregivers in their hospice journey. Responsibilities Serve as the primary point of contact for patients and families, guiding them through every step of the hospice experience. Deliver and document skilled nursing interventions and symptom management based on individualized Plans of Care in the patients’ homes or institutional settings. Conduct initial and ongoing patient assessments, monitor conditions, and adjust care plans as needed. Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to develop and refine personalized care plans. Provide emotional, physical, and spiritual support to patients and their loved ones, offering education, comfort, and bereavement guidance. Teach caregivers safe and confident caregiving skills, delivering practical training and emotional reassurance. Communicate significant updates in patient condition to the care team, ensuring timely and appropriate interventions. Maintain accurate, up‑to‑date clinical documentation in compliance with infection control and regulatory standards. Support quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient. Help patients and families understand and navigate medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes. Minimum of one (1) year of RN experience in acute care hospital, oncology, long‑term care, hospice, or home health. Hospice or palliative care experience preferred. CPR certification. Valid driver’s license and current automobile insurance. Knowledge of terminally ill patients, hospice roles, and community resources. Strong patient assessment, documentation, and program management skills. Understanding of managed care principles and regulatory guidelines (Medicare, Medicaid, ACHC).
