Field-Based Community Health Navigator

Yamhill Community Care

Yamhill Community Care in McMinnville, OR, is looking for a Community Outreach Specialist. This position involves working with Members to help them navigate healthcare services and improve their health outcomes. The role includes both field and office work with a hybrid model. The ideal candidate should have experience with high-risk populations and demonstrate strong communication and relationship-building skills.
