It is the largest of the four Duke Health hospitals with 1,062 patient beds. The facility features comprehensive diagnostic and therapeutic services, including a regional emergency/trauma center and an endo‑surgery center. Position: Transplant Coordinator – Duke University Hospital The Transplant Coordinator works within the Post Lung Transplant Department to provide education, assessment, and ongoing coordination for transplant candidates and recipients. The role requires direct patient contact in clinic and virtual settings and shares on‑call responsibilities for organ offers. Responsibilities Educate patients and families about all aspects of transplantation care, including arranging follow‑up appointments, labs, and coordinating with the multidisciplinary team during the post‑transplant phase. Communicate with patients and caregivers regarding the plan of care and ongoing changes. Maintain a case load of post‑transplant patients to ensure timely care. Provide continuity of care between dialysis, outpatient care, and long‑term management. Make clinical judgments based on current research, principles, and outcomes to solve complex problems. Collaborate with medical faculty, pharmacy, nursing personnel, case management, and other health disciplines to meet patient and family needs. Stay current on transplant research and apply evidence‑based recommendations to patients and families. Complete regulatory documentation in the UNOS database/hospital EMR. Review educational materials and work with the department on updates to practice. Manage immunosuppression plans per protocol and medical team; provide ongoing education to patients and caregivers. Knowledge, Skills, and Abilities Attention to detail. Effective communication skills. Ability to multitask and prioritize multiple demands. Patient‑centered and family coaching skills. Ability to work in a high‑stress environment. Work Hours and Shifts Full‑time: Day shift, on call with some weekends and holidays. Salaried. Minimum of 5 years of nursing experience, with at least 2 years in transplant, critical care, or disease‑specific settings. Licensure, Certifications Current or compact RN licensure in the state of North Carolina required. Basic Life Support (BLS) required.
