We are hiring a compassionate RN or LPN (Nurse) Clinical Care Coordinator/Charge Nurse to provide support to our nursing department. Make a difference with time to truly care.

What We Need You To Do:

• Coordinate and oversee resident care, ensuring quality clinical outcomes through assessment, care planning, medication administration, documentation, and communication with residents, families, physicians, and staff.
• Provide leadership and supervision to nursing support staff while maintaining compliance with facility policies and healthcare regulations.

Key Responsibilities:

• Lead and support nurses and CNA's throughout the shift
• Assist with assessments, rounding with physicians/APRN's and clinical follow-up
• Collaborate with providers, therapy, and interdisciplinary teams
• Promote a positive, accountable, and respectful work culture
• Ensure documentation accuracy and regulatory compliance

These Are The Qualifications We Are Looking For:

• Graduate from an accredited school of Nursing
• Licensed by the Kentucky Board of Nursing as a registered nurse or Licensed practical nurse
• Current CPR certification

• Options - 6:30am - 3:00pm full-time rehab unit
• Options - 6:30am - 7:00pm full-time long term care unit
