RN Navigator for Incoming Cardiology Referrals and Procedural ReferralsMonday - Friday Days 5100 West Taft Road Suite 2U Liverpool, New YorkDuties to include chart compilation, triage/prioritizing referrals, medication review, lab reviews. Works directly with referral specialists for better patient throughput in our cardiology offices. Spanning Camillus, Liverpool, Fayetteville, Brittonfield, Watertown, Rome. Travel between Syracuse office locations on a regular basis. We encourage job share opportunities to enhance satisfaction and growth.Work/Life: Scheduling options balance work/life/school calendars.Safety: Only local hospital to receive a Leapfrog A for safety.Accredited and rated highly for multiple specialty services.What you will do:Manages the case load of patients and identify new patient candidates.Communicates with management on all relevant operation and patient issues.Improves health care access, promotes client knowledge and compliance with behavior modifications associated with diagnosis and treatment regime.Communicates with external offices and patients regarding testing and procedural appointments.Assists with needed referrals for testing and treatment.Presents matrix reporting to accrediting agencies, service line leadership and program providers.Serves as a resource and consultant for best practice information as it relates to the program and services available within our network.Effectively educates the care team, including patient and their support, in medical management and quality of life matters related to diagnosis.Participates in the development and evaluation of the associated care path and educational materials.Provides focused attention on Service Line specific initiatives to include patient satisfaction, adherence to medical plan, re-admission rates, and length of stay.Coordinates and maintains readiness for compliance with regulatory agencies.Supports effective case management to maximize healthcare outcomes, facilitate wellness and identify/correct service delivery complications.Assesses, plans, implements, and evaluates the oncology patient pathways by focusing on knowledge, attitudes, and skills. Development and participation in the education of patients and caregivers throughout the continuum of care.Responsibilities:Collecting, storing and reporting data collection for the development and quality improvement of the program.Develop plan and advance the programs to meet, exceed and/or improve metrics.Manages associated specialty clinics.Participates in and applies knowledge of research, current concepts and guidelines to practice.Complies with insurance requirements for disease management, assists with coordinating care of the uninsured/unassigned patient referrals.Collaborates with service line to provide formal feedback for registries and governing bodies through performance development process.Collaborates with service line to achieve unit and organizational quality improvement and outcome-based initiatives.Provide information as program ambassador to represent the service and promote the program.Works closely with Service Line management regarding daily operational issues, including short, mid, and long-range planning and coordination of the overall service line with a focus on patient care within the continuumEducation, Training, Experience, Certification and Licensure:Bachelor's Degree in Nursing required with three years' progressive experience in the related field preferred. Certification/licensure as required by regulatory agency. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
