Reston, VA Pay From: $40 per hour MUST: BSN required 3+ years of nursing experience, preferably in community health, public health, primary care, ambulatory care, or a similar setting. Previous supervisory, charge nurse, team lead, or clinical leadership experience preferred. Strong knowledge of patient care coordination. Excellent communication, organization, and leadership skills. Ability to work effectively with patients from diverse backgrounds and with multidisciplinary teams. Strong clinical judgment and problem-solving abilities. Proficiency with electronic medical records and standard computer applications. Must be a Registered Nurse licensed in the state of Virginia Must enjoy working in Community Health Must have BLS certification Must be able to travel between multiple locations Team player, compassionate, attention to detail

Duties:

• Supervise, lead, and support RNs, LPNs, and other clinical staff.
• Provide clinical guidance, education, and mentorship to nursing team members.
• Oversee daily nursing operations and help ensure appropriate staffing and patient coverage.
• Coordinate care for patients with complex medical and social needs.
• Conduct patient assessments and assist with the development and implementation of individualized care plans.
• Monitor patient outcomes and identify opportunities to improve quality of care.
• Collaborate with physicians, advanced practice providers, social workers, case managers, community organizations, and other healthcare professionals.
• Help connect patients with appropriate community resources, healthcare services, and social support programs.
• Monitor compliance with nursing standards, organizational policies, and applicable regulations.
• Participate in quality improvement initiatives, audits, and performance improvement activities.
• Assist with onboarding, training, competency assessments, and performance management of nursing staff.
• Address clinical concerns and provide appropriate follow-up and escalation when needed.
• Maintain accurate and timely patient documentation.
• Promote patient education, preventative care, wellness, and chronic disease management.
• Participate in outreach and community health initiatives as needed.
• Serve as a clinical resource for staff and help resolve patient-care issues.
• Perform other duties as assigned.
