What You’ll Do Deliver hands-on occupational therapy interventions in patients’ homes under the guidance of a licensed Occupational Therapist Help patients regain independence with activities of daily living (ADLs) like dressing, bathing, and mobility Track progress and provide real-time feedback to adjust care for the best outcomes Educate patients and families on adaptive techniques and equipment that make everyday life easier Maintain accurate, timely documentation to ensure continuity of care Collaborate with a multidisciplinary team committed to exceptional patient outcomes What You Bring Active Certified Occupational Therapy Assistant (COTA) license Current CPR certification Valid driver’s license & auto insurance Ability to work independently across different home environments Strong communication skills and a patient-first mindset Preferred: Home health or SNF experience Familiarity with HomeCare HomeBase (HCHB) Physical Demands Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
