Nurse Practitioner

Nurse Practitioner (Hospice)

Conducts face-to-face visits with hospice patients in their homes to assist the hospice team in determining whether the patient is still eligible for continuation of hospice services.

Essential Functions:

• Assesses patient's physical condition upon recertification and as needed in accordance with physician's orders, plan of care and Self-Help Home Health & Hospice policies and procedures.
• Initiates communication with attending physician, Hospice Medical Director, and hospice team members as needed to coordinate optimal care and use of resources for the patient and/or patient's family.
• Obtains data on physical, psychological, social and spiritual factors that may influence patient's or family's health status.
• Provides a detailed written narrative when clinical findings support a life expectancy of six months or less.
• Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
• Maintains accurate and timely documentation of all care delivered and ensures compliance with local, state, and federal statutes and regulations.
• Performs other duties as assigned.

Qualifications:

• Current California Licensed Nurse Practitioner.
• One year of prior professional nursing experience. Home health, oncology or hospice experience preferred.
• Must have understanding of hospice principles of care and philosophy and familiarity with hospice requirements.
• Current CPR certification.
• Must pass health screening for TB clearance, physical examination and background check.
• Bilingual in English and Cantonese preferred.
• Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
