Highlights Location: Woodland Park, NJ Facility: Hackensack Meridian Health Schedule: Full-time What You'll Do Provide professional nursing care to patients in their home. Assess, develop, implement, and evaluate home nursing care needs. Ensure compliance with physician orders and documentation. Demonstrate holistic care for patients and significant others. Complete nursing assessments and medication reconciliations. Collaborate with physicians on the Plan of Care. Update care plans within two weeks of changes in patient status. Act as a liaison among physicians, management, and healthcare team members. Supervise para-professional home health aide staff. Maintain awareness of new standards and incorporate them into practice.
