Key Responsibilities\n- \n
- Conduct comprehensive clinical assessments to evaluate the mental health needs of patients facing terminal illnesses. \n
- Develop individualized treatment plans utilizing solution-based approaches to empower patients and their families during their hospice journey. \n
- Implement evidence-based therapeutic interventions tailored to each patient’s unique circumstances, including grief counseling and psychosocial support. \n
- Maintain meticulous clinical records in compliance with legal and regulatory requirements, ensuring continuity of care across healthcare providers. \n
- Collaborate with multidisciplinary teams, including physicians, nurses, and spiritual care providers, to coordinate holistic patient care. \n
- Facilitate case consultations and participate in peer review meetings to share insights and continuously improve care quality. \n
- Provide educational resources and support to patients and their families, enhancing their understanding of mental health and coping strategies. \n
- Stay informed about the latest research and best practices in hospice care and psychotherapy to enhance patient outcomes. \n
In this role, you will primarily work with patients who are experiencing life-limiting illnesses. You will provide crucial support to individuals and their families, fostering emotional well-being during challenging times. Your work will not only help improve the quality of life for these patients but also support their loved ones as they navigate the complexities of hospice care and bereavement.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited institution is required. \n
- A valid LCSW license in the state of Virginia. \n
- A minimum of three years of clinical experience, with a focus on psychotherapy and hospice or palliative care. \n
- Proficiency in solution-based psychotherapy techniques and crisis intervention. \n
- Excellent communication skills and the ability to engage effectively in a collaborative team environment. \n
