Registered Nurse

Accountable to: Administrator/D.O.N. (Program Director in the absence of the Administrator) Qualifications General – Certified by a Nevada licensed physician within 6 months prior to employment to be in good health including freedom from communicable disease, including tuberculosis in the communicable form, according to acceptable screening methods of the Department of Health and Human Services and BHCQC. Education/Profession/Experience – Must have an active Nevada RN or LPN license with at least 1 year experience in a licensed health care facility (preferably a Geriatric Health Care Facility and Intellectual Disabilities). Abilities Understand the needs of the elderly and, in particular, the needs of those affected by cognitive impairments and intellectual disabilities. Be sensitive to the needs of family caregivers. Demonstrate good communication skills. Be available to work unscheduled days and hours. Make a commitment to cooperative performance to attain care, organization and programmatic objectives which may entail performing duties not included in this job description. Responsibilities of the RN or LPN regarding participant health care Work closely with Director of Marketing/Intake Coordinator/Service Coordinators (DRC) in processing new client admissions. Upon receipt of a current PEF, develop an Individual Plan of Care for each client and quarterly thereafter based on the information on the PEF and Intake Questionnaire. Send reminders to clients/families/Social Workers when annual renewals & authorizations are due (i.e. PEF, TB skin tests, CHIP/Clark County/VA Home Assist authorizations). Generate a Referral Form (Medicaid clients only) for new admissions and submit all other required documents for enrollment to Medicaid. Repeat this process timely on an annual basis prior to the expiration date of authorizations, if needed. Provide nursing care based upon physician’s orders and IPC’s, as well as performing assessments of each participant’s health status whenever there’s a change of condition. Within the first hour of admission, 30 days after admission and quarterly thereafter, perform timely Nursing assessments on all clients. If a client is unable to give accurate history, information may be obtained from a family member via phone. Performing an assessment during the intake process will be acceptable provided the start date is no later than 30 days after. A more current assessment will be required if the start date has been delayed of more than 30 days since performed. Provide observation, monitoring and intervention for unstable medical conditions or any changes in condition. Act as a liaison between the client and the participant’s attending physician and consulting physicians, notifying them of any changes in the health status of a participant. Record daily attendance, treatments, vital signs, medication administration, etc. on the Nursing Flowsheets and Progress Notes, and DRC forms if applicable. Communicate with the family caregiver regarding problems related to care of the participant at the center or at home ranging from changes in the participant’s condition or physician’s orders to concerns regarding the participant’s hygiene or dietary/fluid intake. Provide health education and counseling including nutritional advice regarding the participant to family caregivers. Administer, or supervise, as prescribed by the participant’s attending physician, the pharmaceutical regime for each participant, document same and observe for adverse reactions. Provide emergency care including notification of ambulance, physician, emergency care facility, and participant’s emergency contact. Supervise the provision of meals and the participant’s dietary and fluid intake. Coordinate the provision of other ancillary health services (Podiatry, Audiology, Optometry, PT/OT/ST, Home Health, Pharmacy, Mental Health, etc.) provided within and outside of the center, if needed. Make periodic follow up phone calls to families whenever hospitalization or illnesses occur causing absences for more than 3 consecutive days and document. Supervise CNA/Program Aides/Direct Support Staff in obtaining routine vital signs & prn (trained PA's only otherwise RN/LPN) and obtaining monthly weights, if authorized by client or responsible party. Vital signs are to be taken daily during the first 2 weeks of admission, then minimum of 3 days per week and as needed on clients with diagnosis of HTN, CAD, MI, CVA, CRF. Other clients’ VS may be taken a minimum of 1x/wk if stable. JDT clients may be taken 1x/mo, unless has diagnosis of any of the above. Supervise CNA/Program Aides/Direct Support Staff in the provision of restorative or rehabilitative therapy with regards to assisting with the Activities of Daily Living. Supervise CNA/Program Aides/DSS in the provision of prophylactic skin care to avoid breakdown. Supervise CNA/Program Aides/DSS in the provision of direction in assisting with personal care; incontinency care and maintenance of supplies related to same. Train nursing staff and supervise all staff in the use of standard protocols for communicable disease and infection control (including OSHA regulations). Document all nursing services in the participant’s medical record, including all communications with family members, Social Workers & Physicians. Manage participant medical records including periodic review for content and completeness. Periodically inspect drug storage area, treatment supplies, emergency equipment and personal care supplies & discard ALL expired items.
