Compassionate Hospice RN Case Manager
UnitedHealth Group
Saint Mary's Hospice, part of the UnitedHealth Group family, seeks a Registered Nurse Case Manager to oversee the Plan of Care for each hospice patient. You will evaluate care needs and collaborate with the care team to promote physical, psychosocial, and spiritual well-being in home settings. The role requires an active RN license, valid driver's license, CPR certification, and the ability to coordinate comprehensive care, document progress, and support patients and families through challenging #J-18808-Ljbffr
