Job Description: Summary: The Team Member Health Licensed Practical Nurse is responsible for the evaluation and promotion of team member health and safety through screening of team members. Supports and assists in the promotion, education, and distribution of Ballad Health's preventive and wellness programs. Assist in the evaluation, education, counseling, and follow-up of team members having exposure to communicable disease. This position is essential in assisting in areas where compliance with federal, state, and accreditation standards are necessary. The Team Member Health Licensed Practical Nurse evaluates counsels, motivates, and refers team members with various physical and emotional concerns. Demonstrates competence in evaluating, interpreting, and referring work related team member results. This person assists in maintaining health records in a confidential manner and in accordance with all governing agencies. This position demonstrates competence to treat and care for adult patients, adolescent to 99 years of age.

Requirements: Must be a graduate of an accredited school of Licensed Practical Nurse. At least 5 years of LPN clinical experience is required. Good physical assessment, communication, and counseling skills are essential. Previous team member health and/or infection control experience is preferred.

Licenses and Certifications: Current licensure in the state of Tennessee
