Care Coordination Case Manager

UF Health is seeking a Case Manager to participate in creating the patient care plan and to promote efficient throughput across the care continuum. The role includes discharge planning, utilization review, and collaboration with patients, families, and the care team in a busy Florida hospital setting. The ideal candidate has RN licensure in Florida or an equivalent MSW/BSN background, with 1–2 years in discharge planning or case management, and preferred certifications in case management. #J-18808-Ljbffr UF Health
