About the Role:

North Point Senior Homecare in Virginia Beach, VA is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team on a PRN basis. Responsibilities:

• Provide direct patient care to senior clients in a home care setting under the supervision of a Registered Nurse or physician
• Administer medications, treatments, and injections as prescribed and document accordingly
• Monitor and record patient vital signs, health status, and changes in condition
• Assist with wound care, catheter care, and other skilled nursing procedures
• Communicate effectively with patients, families, and the care team regarding patient progress and needs
• Maintain accurate and timely clinical documentation in compliance with state and agency regulations
• Educate patients and caregivers on health management, medications, and care plans

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of Virginia
• Previous home care, geriatric, or long-term care nursing experience preferred
• Current CPR/BLS certification
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with a patient-centered approach
• Reliable transportation and valid driver's license for home visits
• Ability to work independently with minimal supervision in a home care environment
