What You'll Do:

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders
• Develop and implement personalized treatment plans based on standardized assessments and evidence-based practice
• Monitor patient progress and adjust treatments to optimize outcomes
• Provide counseling and education to patients, families, and caregivers
• Collaborate with healthcare team members to coordinate care and ensure regulatory compliance
• Document all services accurately in the electronic medical records system
• Support supervision of CFY or student Speech-Language Pathologists as needed

What You Need:

• Active ASHA and State Licensure as a Speech-Language Pathologist
• Ability to obtain necessary DME or supplies
• Knowledge of ASHA and State practice acts, ethics, and standards

• Previous experience in long-term care settings
• Strong communication and team collaboration skills
