Registered Nurse (RN) - Case Management

Case Manager – Acute Care

Variable 5-day workweek; department operates 7 days/week. Days off will vary and are not guaranteed to be back-to-back. Weekend requirement is variable.

Daily census may be up to 25 patients with a ratio of 1:25.

Requirements

• Minimum 2 years acute medical care management experience performing care coordination.
• California state license required (pending accepted but must be licensed on start date).
• Epic experience is required.
• Experience with care coordination, discharge planning, and arranging post-acute services is necessary.
• Experience with vulnerable populations including mental health and substance use disorders is preferred.

Responsibilities

• Assess patients and coordinate care to facilitate discharge planning.
• Arrange for durable medical equipment, skilled nursing facilities, and transportation services.
• Participate in family meetings and treatment planning sessions.
• Document patient care and coordination activities in the electronic health record.

Compliance / Notes

Must commit to working 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day. Requested time off must not exceed 7 days and should be requested upfront during submittal.
