Housing Stability Case Manager (HIV Services)

Housing Works in New York City is seeking a Housing Case Manager to provide comprehensive case management and care coordination for residents living with HIV and co-occurring conditions. You will work in clients’ homes, in the office, and in the community to promote housing stability, independent living, financial management, and health and wellness. The role requires in-person engagement, documentation per standards, and advocacy to end homelessness and HIV/AIDS, with a focus on collaborative #J-18808-Ljbffr Housing Works
