Social Worker I - Case Management

Job Summary Social Workers are responsible providing necessary support for each patient referred to their discipline according to health status, diagnosis, and patient and family specific needs. Hospital social workers advocate and help patients understand and navigate complex systems. With compassion and cultural awareness, they empower individuals to make informed decisions during challenging times. Responsibilities Collaboratively works with interdisciplinary team and complex case management team to ensure completion of patient specific care plan and post-acute needs and placement based on patient choice. Effectively collaborates with community resources to align patient with available resources. Assists with referrals for community based resources, as needed. Completes required governmental assessments and level of care documents. Assists indigent population with needed resources to aid in prevention of readmissions and support quality in patient outcomes. Assists with various populations across the organization to ensure proper resources and support including but not limited to APS, CPS, DSS, disability based on identified patient safety and needs. Consistently handles customer complaints promptly and effectively when problems encountered. Utilizes proper escalation procedures when appropriate. Performs all other duties as assigned. Qualifications 1-3 years related experience, computer literacy, excellent communication and writing skills. Licensed Bachelors prepared Social Worker within the associated region of responsibility required within 1 year of hire.
