Job Title: Home Health Physical Therapy Assistant (PTA) – $5,000 first‑year success bonus. Location: Lincolnwood, IL (and surrounding areas including Chicago). Employee type: Full‑time. Working hours per week: 40. At Residential Home Health and Hospice ('Residential'), we’re looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team‑based environment, our Home Health Physical Therapy Assistants collaborate to manage and deliver patient care plans. This position supports patients in Chicago, Lincolnwood, and surrounding areas and is eligible for a $5,000 first‑year success bonus to reward the accomplishment of key achievements during your first year. Responsibilities Collaborate to manage and deliver patient care plans. Consistently meet critical deadlines for charting. Self‑manage time and schedule. Demonstrate experience with tablets, mobile phones and EMR software. Qualifications Associate degree in physical therapy from an accredited college or technical school. Minimum one year of experience in an acute care and/or rehabilitative setting preferred; previous home‑health experience preferred. Must hold, in good standing, a license/registration as a Physical Therapy Assistant issued by the State in which you work. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
