Advanced Home Health Care is hiring a full-time Physical Therapist to provide in-home physical therapy services throughout our Southeast Iowa coverage area, including Des Moines, Lee, Henry, Louisa, and Van Buren Counties.

We offer a $100,000 annual baseline salary, negotiable based on experience , flexible scheduling, mileage reimbursement, comprehensive benefits, and the opportunity to play an important role in our growing internal therapy program.

As a Home Health Physical Therapist, you'll also have greater flexibility and autonomy over your workday . Rather than spending each day in the same clinical setting with back-to-back appointments, you'll work throughout the community, build meaningful relationships with patients and families, and organize your patient visits around their needs and your schedule.

What You'll Do

• Provide direct physical therapy services to patients in their homes
• Monitor patient progress and outcomes
• Adapt recovery plans to each patient's individual needs and goals
• Help patients improve mobility, independence, and overall well-being
• Collaborate with an interdisciplinary healthcare team to provide coordinated, patient-centered care
• Communicate effectively with patients, families, and other members of the care team
• Travel throughout our Southeast Iowa service area to provide patient care

What We're Looking For

Required:

• Valid, active Physical Therapist license
• Relevant clinical experience
• Strong communication and organizational skills
• Collaborative, patient-centered approach
• Commitment to providing high-quality care and improving patient outcomes

Preferred:

• Previous home health experience
• Leadership experience
