Physical Therapist-Hybrid SNF/Homecare

As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program! What is Take Your Therapist Home? Take Your Therapist Home (TYTH) is a unique program where clinicians work directly with patients in skilled nursing facilities (SNF) and home health, providing continuity of care and greater impact on patient outcomes. You will primarily work in the home health setting with dedicated time and caseload support for the SNF. Your coverage area will be localized to the surrounding area of the SNF. Opportunity to expand into other settings. Facility Location Belle Fountain – Hybrid role providing facility‑based therapy services and local home‑care therapy services. Facility services paid hourly; home‑care services paid per visit. Schedule Monday – Friday, full time. General Summary In this role you will provide physical therapy services to patients impaired by physical dysfunction, including assessment, treatment planning, and implementation of treatment plans. You will participate in patient care conferences, program development, and screenings. You will supervise PTAs and aides as assigned and complete appropriate documentation, adhering to policies, procedures, and current clinical practice standards. Your Responsibilities Conduct physical therapy screenings and evaluations to determine patient status. Provide physical therapy treatment consistent with APTA professional standards. Develop and implement treatment plans using strengthening, modalities, and retraining to enable patients’ independence. Provide direct patient care using graded activities to restore physical and functional abilities. Check, instruct, assign, and review work of PTAs and aides. Document to maintain quality of medical records. Participate in patient care conferences per company guidelines. Participate in CQI activities to ensure continuous quality of care. Promote current practice standards through continuing education and in‑service programs. Coordinate or participate in in‑service programs for internal and external groups. Participate in new program development. Attend company meetings and additional activities to facilitate department operations. Required Qualifications Successful completion of an accredited physical therapy program. Licensure required by the State of Michigan. Valid driver’s license. Skills and Abilities Exercise professional judgement and initiative when analyzing problems and recommending solutions. Initiative and flexibility in the work environment. Respond effectively to patients’ and families’ needs. Physical ability to bend, position, transfer, and lift patients weighing 80–300+ pounds, using recommended lifting devices.
