Physical Therapist - Travel Contract

Minimum Requirements
Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active Indiana Physical Therapy license required prior to start of assignment.
BLS certification from AHA or American Red Cross may be required.
Outpatient clinic experience preferred, including evaluation and plan of care development for musculoskeletal and post-operative patients.
Experience with documentation, goal writing, letters of medical necessity, and charge entry in outpatient EMR systems preferred.
Demonstrated ability to supervise and delegate to therapy assistants and students preferred.
Maintain professional licensure and complete state-mandated continuing education as required by Indiana Practice Act.
Assignment Details
Facility Type: Outpatient clinic.
Shift Pattern: 5x8 schedule.
Assignment Duration: 13 week assignment.
Positions Available: 1 position.
Candidate responsibilities include evaluation, development and modification of treatment plans, documentation in accordance with payer and clinic policies, and supervising discipline-specific therapy services.

Outdoor opportunities include proximity to Brown County State Park (about 30 miles south) and Morgan-Monroe State Forest (approximately 20 miles northeast), suitable for hiking and mountain biking. The area offers a suburban community atmosphere with seasonal festivals, community parks, and a short commute to urban amenities in Indianapolis, making it a balanced fit for clinicians seeking a quieter base with city access.
