Responsibilities

• Administer prescribed medications and treatments per physician orders and facility policy
• Monitor resident status, document findings, and promptly report changes to the supervising RN
• Assist with activities of daily living (ADLs) and support resident mobility and safety during night shifts
• Perform routine nursing tasks such as vital signs, wound care, and clinical observations
• Collaborate with an interdisciplinary team to implement individualized care plans
• Maintain infection control practices and ensure resident safety in compliance with regulatory requirements

Requirements

• Active Licensed Practical Nurse license (state-specific as required)
• Current licensure in good standing
• Ability to work night shift and the 3x12 schedule
• Strong communication and teamwork skills
• Proficiency with basic electronic health record documentation and routine nursing tasks
