Your role as an LPN You’ll be a trusted clinical leader on the evening shift, providing high-quality, person-centered care that helps residents rest safely and comfortably. Deliver direct nursing care according to individualized care plans Administer medications and treatments accurately and safely Closely monitor residents, quickly recognize changes, and communicate with RNs and physicians Maintain clear, timely, and accurate documentation Guide and support CNAs to ensure consistent, excellent care Assist with smooth admissions, transfers, and discharges Build trusting relationships with residents, families, and the care team Uphold the highest standards of infection control and resident safety What we’re looking for Active, unrestricted Pennsylvania LPN license Genuine passion for geriatric and rehabilitative nursing Strong clinical skills, sharp attention to detail, and a team-first mindset Commitment to dignity, respect, and person-centered care If you’re a dependable, caring nurse who wants to work where you’re valued — not just scheduled — we’d love to meet you.
