Physical Therapist (PT)

Job Description

The Physical Therapist will be responsible for screening, evaluating, and rehabilitating patients experiencing physical functioning disorders and/or requiring wound care. The therapist will develop and implement appropriate plans of care in accordance with regulatory and clinical practice standards.

Utilize standardized assessments and evidence-based practices to guide clinical interventions.
Administer various rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
Assess patient progress and adjust treatment plans as necessary, ensuring documentation adheres to regulatory and clinical practice requirements.
Evaluate treatment outcomes for effectiveness and modify services based on patient evaluations.
Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Maintain effective communication with supervisory personnel and other health team members regarding patient progress, barriers, and treatment plans.
Compile comprehensive discharge summaries of services rendered in compliance with regulatory and clinical requirements.
Document all necessary information in the electronic documentation system and maintain patient records in accordance with company policy.
Accurately record treatment minutes for services provided in alignment with all regulatory and clinical guidelines.
Participate in required meetings such as patient care conferences and rehabilitation meetings.
Engage in facility and company mandated training sessions, in-services, and conferences.
Obtain necessary durable medical equipment or supplies following regulatory and clinical practice standards.
Supervise and support physical therapist assistants, rehabilitation aides, and students.
Adhere to APTA and state practice acts, code of conduct, and ethics in all professional dealings.

Qualifications

Minimum of 1 year of experience as a Physical Therapist.
Experience in Long Term Care (LTC) or rehabilitation settings is required.
Active Oklahoma State License is mandatory.
