Sustained members of our team demonstrate accountable behavior and share our values of customer service, innovation, integrity, inclusion, financial stewardship, leadership and care.The Hospice Clinical Supervisor leads the hospice clinical team to utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet hospice patient/family physical, emotional, psychosocial, and spiritual needs while coordinating available resources to promote quality and cost-effective outcomes through ongoing audits of medical records and clinical documentation to continually improve data quality in accordance with company standards and federal, state, and local standards, guidelines, and regulationsEssential Activities and TasksOperations Management and Administration - 55%Facilitates clinical team with the coordination and oversight of all direct and indirect patient/family care services ensuring accurate evaluation of patient/family physical, emotional, psychosocial and spiritual needs, and development and implementation of comprehensive care plans that address those needs by following current hospice nursing professional standards of practice and agency approved protocols. Supervises the implementation of the physician ordered hospice plan of care, by coordinating, auditing, overseeing, integrating, and modifying the IDG resources necessary to reach desired outcomes.Oversees Hospice IDG review, revision, and documentation of the individualized plan of care as frequently as the patient’s condition requires, but no less frequently than every 15 days.Organizes and oversees hospice IDG (interdisciplinary group) meetings which are held no less frequently than every15 days.Interprets operational indicators to detect census changes and increases/decreases in volume that could drive staffing levels, revenues, and/or expenses.Evaluates and approves the clinical documentation and approves electronic medical record workflow according to established standards.Reviews diagnosis coding and plan of care orders for all hospice patients in accordance with Medicare regulations.Assures all equipment, systems, and monitoring are in place to meet or exceed quality, customer service, utilization, and compliance standards in all company hospice programs.Collaborates with marketing team to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.Manages the local component of the integrated electronic medical record and assures that the office and clinical staff are well trained and use the system productively and accurately according to company policies.Evaluates the effectiveness of the plan of care for hospice care delivery and collaborates with hospice clinical supervisors and hospice staff on best practice and desired outcomes.Evaluates workflow processes, administrative processes, and departmental processes.Monitors and maintains hospice revenue cycles and collection cycles and ensures accuracy in documentation and coding.Conducts supervised patient visits.May be asked to case manage to meet business needs.Participates in on-call rotation, as needed.May assume the role of Alternate Administrator during the temporary absence and/or vacancy of the Administrator.Quality and Compliance Management - 30%Audits and maintains current and accurate medical records and clinical documentation in accordance with company standards and federal, state, and local standards, guidelines, and regulations.Ensures action is taken to correct problems identified through clinical record review, clinical audits, and fiscal audits.Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.Oversees the state survey processes, mock survey, and compliance audits. Maintains a state of readiness for survey by regulatory bodies.Serves as a resource to clinical team for questions and/or concerns regarding federal and state regulations, approved accrediting bodies, and professional standard of practice.People Management and Development - 15%Supervises the hospice clinical team to operate within the allocated resources and budgeted goals, efficiencies and productivity standards.Fosters a positive work environment that attracts, retains, and motivates employees.Interviews, selects, and orients new direct reports.Defines, establishes, and promotes performance expectations so direct reports understand and demonstrate excellence through high quality and consistent performance.Evaluates job performance of direct reports in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth.Monitors accountability with organization requirements prescribed by the Employee Handbook. Documents and engages employees who are routinely not observing these requirements in an effort to achieve compliance. Works with Human Resources when circumstances arise that necessitate further documentation and intervention.Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff.Ensures that there is a reasonable and sufficient plan for staffing should a direct report be absent or a direct reporting position be vacant.Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval.All other duties as assigned.QualificationsEducationCurrent unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.Bachelor of Science in Nursing (BSN) preferred.ExperienceThree years experience in clinical operations supervision required.One year experience in a hospice setting required.One year experience in a Medicare-certified home health agency required.Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.Proficiency with electronic medical records (EMR) preferred.Other RequirementsMust be able to read, write, speak, and understand the English language.Must possess a valid driver's license and acceptable driving record.
