Case Manager RN

Methodist Healthcare in San Antonio seeks a compassionate Case Manager RN to coordinate care across the continuum for diverse patient populations. In this role, you will perform clinical assessments, develop and update care plans, facilitate safe discharges, and ensure appropriate utilization of hospital and community resources. You will collaborate closely with physicians, interdisciplinary teams, patients, and families to remove barriers to care, improve outcomes, and reduce readmissions.
