Floating Family Services Advocate Job Summary: This position works closely and individually with families to support their success in our center‑based and Learning at Home programs. This floating role requires travel to all schools when needed. The advocate partners with families of enrolled children to facilitate family success in goal setting, child health records, medical home documentation, referrals for emergency support, monitoring compliance requirements, and providing home language support as necessary. Responsibilities include developing and initiating Child Health Plans & Individual Health Development Plans and navigating families through community health services to meet children’s unique health needs in accordance with state and federal regulations (HHS/OECP/OKDHS). The advocate supports family engagement, organizes and promotes family connection events, facilitates group connections and parent programming, manages an assigned caseload, ensures regular touch‑points, and completes required documentation such as the Family Success Plan, health records, and well‑child checks. Responsibilities Family Enrollment Meetings: Provide personalized support to families of newly enrolled children by collecting required documents and discussing expectations regarding attendance and other important program aspects. Create a positive and engaging experience for both children and parents by addressing their unique needs and maintaining strong relationships through ongoing follow‑up. Screening and Documentation Review: Ensure compliance with applicable regulatory requirements for health/dental services, obtain required consents and releases, review medical documentation, perform screenings, assist with examinations, and complete necessary follow‑ups (WCC, Lead, Sensory, Growth, and Dental). Referrals: Refer clients to medical/dental providers, follow up with providers and families as necessary, and coordinate financial assistance plans for families unable to pay for services. Record‑Keeping: Submit health screening results to parents, schools, and partners; electronically attach and track confidential medical records in the ChildPlus database, state immunization and lead databases, and other applicable record‑keeping databases. Compile weekly and monthly reports on screening, exams, and follow‑up progress. Special Health Needs: Implement and oversee care plans for children with special health care needs, monitor health records, and contact parents regarding due dates or restrictions. Family Success Plan (FSP) & Referrals: Assess family strengths and needs, complete Family Success Plans, collaborate with families to develop individualized goals, provide emergency referrals, and follow up with families after absences. Parent Programming and Involvement: Encourage and support parent involvement in Parent Connections, male involvement initiatives, parenting skill classes, Policy Council recruitment, and elections; organize and promote family connection events and provide resources on community events. Home Language Support (if applicable): Assist parents with screenings, assessments, interpretation services during visits and meetings, and participation in multidisciplinary team meetings. Perform other duties as assigned or requested. Education & Experience Associate’s degree, preferably in social work or a related field (human services, family services, counseling), or equivalent work experience. Bachelor’s degree preferred. CFLE (certified family life education credential) within 18 months of hire unless a related degree is held. 2 years of related work experience. Bilingual (English/Spanish) or (English/Zomi) preferred. Certification / License Must obtain a food handlers permit and CPR/First Aid Certification within 30 days of employment. Must obtain state certification for vision screening and medication administration within one year of employment. Knowledge / Skills / Abilities Proven interpersonal relationship skills to maintain effective relationships with staff, vendors, clients, visitors, and contractors. Able to work collaboratively and respectfully with families. Able to follow oral and written instructions and present information efficiently. Able to accurately audit records and ensure compliance with guidelines. Able to take initiative, multitask, and work well under pressure. Must maintain strict confidentiality of sensitive data and child/family information. Intermediate MS Office skills and technical skills for data entry, report generation, and navigation of systems such as ChildPlus. Advanced organizational and detail‑oriented skills with a commitment to timely completion of assignments. Understanding of Head Start performance standards and OECP program requirements. Essential Physical & Mental Requirements Employee is required to stand, stoop, walk, lift, and carry objects throughout daily performance. Must be able to lift and carry fifty (50) pounds.
