Location & Travel Must reside in Miami Dade County, zip codes 33184, 33172, or 33186. Requires 50%–75% travel for face‑to‑face member visits. Schedule: Monday–Friday, 8:00 am–5:00 pm (standard business hours). Responsibilities We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our Case Management team. This role involves telephonic and face‑to‑face assessment, planning, implementation, and coordination of all case‑management activities with members. The coordinator evaluates medical needs and facilitates overall wellness, managing a caseload of supportive and medically complex members. They proactively develop action plans to address issues, enhance short‑ and long‑term outcomes, and integrate services and supports such as prior authorizations, coordination with PCPs and skilled providers, condition management information, medication review, and community resources. Required Qualifications Bilingual in English and Spanish. Reside in zip codes 33184, 33172, or 33186. Willing and able to travel 50%–75% of time for face‑to‑face visits. 1+ year experience in behavioral health or long‑term care setting. Computer proficiency in Microsoft Word, Excel, and Outlook. Preferred Qualifications Previous field case‑management and discharge planning experience. Ability to multitask, prioritize, and adapt to a fast‑paced changing environment. Effective verbal and written communication skills. Managed Care experience. Education Bachelor’s degree required; preferably in social work or related field. Weekly Hours 40 hours per week. Time Type Full time. Pay Range The typical pay range for this role is: $21.10 – $40.90 per hour.
