Community Living Support Specialist

Job Description: Provides supervision and necessary personal assistance services as specified in the participant's person-centered service plan.Role and ResponsibilitiesFacilitate independence and promote integration into the community for a participant residing in his or her own home or in his or her family's home.Completion of paperwork and other necessary documentation (such as medication logs).Appropriate communication among staff, family, and other agency staff.Medications – assure that clients receive the appropriate medical services, i.e., obtaining medications (fill prescriptions), schedule physician appointments, and give medication, documentation of these services.supports and assistance that shall be related to chosen outcomes, not be diversional in nature, and may include:Routine household tasks and maintenanceActivities of daily livingPersonal hygieneShoppingMoney managementMedication managementSocializationRelationship buildingLeisure choicesParticipation in community activitiesQualifications and Education RequirementsHigh School Graduate or GED.Extensive on-the-job training.Capable of On-line Computer trainingPhysical RequirementsCapability of lifting and positioning clients when necessary.Special RequirementsValid driver’s license is required.Ability to work flexible schedule, including weekends, evenings, overnights, or holidays.Qualifications and Education RequirementsHigh School Graduate or GED.Extensive on-the-job training.Capable of On-line Computer training #J-18808-Ljbffr
