Hourly Pay: $35 - $36 / Hour Maxim Healthcare is hiring for a Licensed Practical Nurse (LPN) to work on our Transition team. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim. Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-30 days, with a 14 day grace period. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then transitioned to another new admission.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN in the state in which the LPN will practice.
• Competency with enteral feeds, tracheostomies, and ventilators.
• Competency with adult and pediatric patients
• Current TB or Chest X-Ray.
• Current BLS card.
