Case Manager PRN

The Case Manager is responsible for coordinating and monitoring day-to-day operations of the Case Management Utilization Management programs. He/She directs the activities necessary to ensure appropriate utilization of the Hospital and its resources while maintainig optimal achievable standards of patient care. Position Requirements: LVN or RN, graduate from an accredited school of nursing Must hold a current state licenseand must maintain license renewal in accordance with the standards of the State Board of Nursing Minimum of three years of full-time management experience, with two years experience in the area of Case Management Additional education in the area of Quality Management and Joint Commission Standards Long term acute care and rehabilitation experience is preferred Certification in Case Management (CCM) preferred Must be able to read, write, and speak English Consumer Access to Health Records & Complaints In accordance with Texas law, Mid-Jefferson Extended Care Hospital provides the following information to help consumers access their health care records and raise concerns or complaints.
