Community Health Nurse

As a Community Health Nurse, you will perform the following duties: Community Assessment: Review county health data to identify needs and guide public health activities Program Development: Create initiatives based on protocols, guidelines, and community priorities Health Monitoring: Track communicable diseases and enter data into state health systems Public Education: Provide information on health topics to support community awareness Outreach Services: Offer case management and support to individuals and targeted groups Clinical Care: Deliver immunizations and conduct disease‑related specimen collection according to guidelines Interested in learning more? Work Schedule and Additional Information: Full‑time employment, 37.5 hours per week Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30‑minute lunch. Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy. QUALIFICATIONS

Special Requirements This position requires active authorization to practice as a Registered Nurse in Pennsylvania. All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing Other Requirements You must meet the PA residency requirement.
