PRN Hospital Pharmacist

Cardinal Health Pharmacy contributes to Cardinal Health Pharmacy Operations by coordinating pharmacy services for acute care hospitals, hospital retail customers, ambulatory care, alternate site facilities, oncology and cardiology practices, and retail customers. The role is based at PAM Health Rehabilitation Hospital of Wiregrass Ranch in Wesley Chapel, FL. Job Summary We are hiring a PRN Pharmacist to provide pharmaceutical care during facility hours (Monday – Friday, 8:00 AM – 4:30 PM) and on-call after hours, including weekends and holidays. Responsibilities include oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy, along with drug information and system oversight. Responsibilities Interpret medical orders (verbal and written) and transcribe to patient medication profiles accurately. Maintain complete and accurate patient medication profiles. Compound and dispense pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products. Issue controlled substances to patient care areas and maintain required records. Monitor drug therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, allergies, and overall appropriateness. Assist with pharmacokinetic consult service and drug dosing per protocol. Review culture and susceptibility data to recommend appropriate antibiotic changes. Detect and report suspected adverse drug reactions promptly. Sustain the formulary by minimizing non‑formulary procurements and promoting therapeutic substitution. Provide clinical consultation and clarification to practitioners. Suggest cost‑effective therapeutic alternatives to medical staff. Document all clinical activities and interventions accurately and completely. Participate in quality improvement and medication use review activities. Collect data, conduct quality monitors and inspections, and maintain logs and records. Participate in orientation, education, and training programs for pharmacy, medical, nursing, and other staff. Perform other duties as assigned. Qualifications Bachelor’s degree in Pharmacy or PharmD. Licensed and in “good standing” as a pharmacist in the state of Florida. In‑patient hospital pharmacy experience preferred. IV experience preferred. Flexibility to work various shifts, including on‑call rotations. Strong oral and written communication skills. Strong customer service skills. Manual dexterity for occasional reaching, lifting, and holding of small objects. Computer savvy to learn and operate multiple pharmacy information systems. May require vendor credentialing.
