Bilingual Behavioral Health Case Manager

Orlando Health Physician Associates seeks a Case Manager to support adult and adolescent patients within the care continuum. You will assess needs, coordinate referrals, advocate for patients, and participate in community outreach to promote health and wellness. The role requires a Bachelor’s degree in a healthcare-related field and at least one year of relevant experience. A valid Florida driver's license and insurability are required for travel as needed. #J-18808-Ljbffr
