ICMS Case Manager

Position Summary Under the direction of the Manager of Housing Based Support Services, the ICMS Case Manager will be responsible for providing coordinated case management and supportive services for formerly homeless individuals with special needs. The manager will deliver on‑site services to tenants residing in our newly developed multi‑unit permanent supportive housing complex and will help link clients to services including, but not limited to, life‑skills workshops, money management, educational attainment, access to health services, counseling, job training and placement opportunities, and resident activity coordination. Responsibilities Conduct initial housing intake, needs assessment, and a minimum of two face‑to‑face contacts with each tenant monthly. Help tenants set personal goals and develop Individual Service Plans (ISP). Assist in overcoming pre‑existing conditions and obstacles to ensure timely completion of all housing‑related processes. Host quarterly meetings to review tenant progress as evidenced by Individualized Stability Plans. Act as a liaison among the Property Manager, the property management company, the tenant, and the primary mental‑health provider to address concerns that could jeopardize residency. Provide education on Fair Housing, tenant rights and responsibilities, and compliance with rental agreements and property rules. Coordinate linkage to public social service agencies and community providers to support employment, education, money‑management, legal, and benefits counseling. Assist clients in obtaining rental assistance, including move‑in/eviction‑prevention services. Address tenant grievances, mediate resident disputes, and resolve issues with property management that might affect tenancy. Collaborate with property management to enforce housing rules. Work with the Resident Services Coordinator to develop workshop opportunities on housekeeping, money management, parenting, daily living skills, transportation, and self‑advocacy. Collect and enter data into the DHS CHAMP database, assemble reports, and evaluate program outcomes. Organize and manage client files. Monitor status of tenants ready to transition to mainstream non‑supportive housing. Comply with documentation and reporting requirements per the service contract. Compile and submit monthly progress reports tracking tenancy status and supportive services delivered. Uphold CRCD’s Mission Statement and 5‑Year Strategic Plan. Requirements 1–2 years of relevant experience or a BA Degree. Experience working with youth experiencing homelessness, housing instability, foster care, probation, or mental health systems. Reliable, flexible team player who thrives with minimal supervision, demonstrates a strong work ethic, and sets and maintains personal boundaries. Reliable transportation: valid driver’s license and car insurance as required by law. Proficient in Microsoft Office suite. On‑site position: report to the office five days a week (not a work‑from‑home role). Salary $50,000.00 – $55,000.00 annually. Benefits 14 paid holidays. On‑demand training memberships.
