Home Care Liaison

Addus Family company

Addus Family Company is seeking a Home Care Liaison to grow and support our in-home care services. In this role, you build and manage referral relationships with hospitals, clinics, and community partners, educating them on Addus HomeCare’s personal care, home health, and hospice services. You will assess patient needs, coordinate smooth transitions to home care, and ensure timely, accurate onboarding. Working closely with clinical and caregiving teams, you promote quality, compliance, and patient satisfaction while representing our mission-driven, compassionate culture in the community.
