RN Case Manager: Discharge & Care Coordination

CareWell Health

CareWell Health in New Jersey seeks a Registered Nurse to provide comprehensive case management for hospitalized patients, coordinating discharge planning to maintain safety and quality of life. You will assess patient needs, review admissions, and liaise with home health, community agencies and payers to arrange equipment and services for discharge. BSN preferred; Medical/Surgical, Critical Care or Community Health Nursing experience required. #J-18808-Ljbffr
