Patient Care Manager (Hospice)

Reasonable Accommodations may be made to enable qualified individuals with disabilities to perform essential functions.Job Responsibilities:Review available patient information related to the case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to the case as needed. Conferences with Medical Director regarding any questions about an individual's eligibility for services.Reviews and evaluates each case through a variety of means such as home visits, conferences, record review, and the services provided by clinicians; discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.Reviews the patient's medical diagnosis, prognosis, medications, procedures, and clinical course.Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.Assists in the screening and interviewing process of new employees and makes recommendations for the employment of individuals. Assists in the orientation of new employees.Assists the Director of Clinical Operations and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.Assesses health needs of the community and assists in the development of the plan(s)/program(s) to meet needs. Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.Promotes customer service orientation to all hospice personnel.Participates actively in quality assessment performance improvement program.Participates in the on-call rotation as needed.Performs other duties and activities as delegated by the Director of Patient Care and/or Administrator.Knowledge of and adherence to all policies and procedures.Maintains compliance by documenting accurately and timely within the organization's electronic health record.Lives the Mission, strives to achieve the Vision, and exemplifies the Core Values of the organization.Works toward exceeding chosen Pillar goals in the areas of Employee Satisfaction, Customer Satisfaction, Quality Service, Compliance, Growth, and Financial Performance.Knowledge of and adherence to the employee Code of Conduct and Code of Ethics.Education and Experience:Registered nurse with current licensure with no restrictions to practice professional nursing in the state.Must have two (2) years of experience as a Registered Nurse.Previous experience in hospice/home care setting and with two years of management or supervisory experience.Maintains current CPR certification.Computer experience including knowledge of Microsoft Suite and/or Google Suite.Other Physical Requirements:Ability to wear Personal Protective Equipment (PPE) (Medical Mask)Work Environment:Primary office work with fieldwork visiting patients in their homes, physician offices, hospitals, hospices, nursing homes, etc.Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
