Nurse Practitioner

Healthonomic Primary Care is a growing, physician-led practice in Chicago's southwest suburbs, and we're looking for a Nurse Practitioner to join our team. About the Role You'll care for an established panel of adult patients, managing chronic conditions, preventive care, and wellness visits. You'll work closely with our physician and a supportive team of MAs and front-office staff. Responsibilities Care for an established panel of adult patients. Manage chronic conditions. Qualifications Required Skills Primary care, internal medicine or family practice experience preferred. Someone who enjoys long-term relationships with patients and wants to help shape a growing practice. Epic EHR. Primary care experience. Internal medicine experience or Family practice.
