QMHP-CS: Community-Based Mental Health Specialist

Position Summary: The Qualified Mental Health Professional (QMHP-CS) provides rehabilitative services and targeted case management to support individuals with mental health needs. This role focuses on developing coping strategies, improving functioning, and fostering independence in home, school, and community environments to the client in accordance with the treatment plan that is designed to meet the mental health, educational, vocational, residential, financial, social, etc. needs of the consumer. Key Responsibilities: Client Assessment & Intervention Deliver therapeutic interventions to enhance client functioning and interactions. Conduct CANS/ANSA assessments, develop and update individualized treatment plans. Provide structured, face-to-face sessions based on treatment goals. Implement skill-building and rehabilitative strategies to support daily living and self-management of symptoms. Service Coordination & Crisis Response Assist clients in identifying triggers, coping mechanisms, and social support. Provide crisis intervention services in line with agency protocols. Collaborate with psychiatrists, therapists, families, schools, and community resources. Ensure services are primarily community-based, with limited clinic-based sessions. Documentation & Compliance Complete progress notes, treatment goals, and related documentation within 48 hours of service delivery. Maintain accurate, timely records in compliance with State of Texas standards. Participate in clinical supervision and attest to completion of mandatory training. Team Collaboration & Program Participation Maintain a caseload depending on agency assignment. Attend monthly staff meetings, team consultations, and training sessions. Support continuity of care as an integral member of the clinical team. Qualifications: Required: Bachelor's Degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention; Master's Degree a plus Demonstrated competency in MHTCM and MH rehabilitative services. Preferred: Familiarity with Medicaid service documentation. Skills & Competencies: Strong interpersonal and communication skills. Ability to establish rapport and maintain professional boundaries. Organizational skills for scheduling and documentation. Adaptability and teamwork skills. Commitment to ethical, client-centered service. Work Conditions: Flexible schedule with some evening and weekend availability. Travel required to client homes, schools, and community locations. Must have an insured vehicle, valid driver’s license, cell phone, and computer/laptop. Position may involve sitting, standing, walking, and occasional lifting (up to 25 lbs).
