Title Nurse Practitioner, Advanced Practice Provider Company Oak Street Health Location 4400 Dorchester Rd Ste 117, Charleston, SC 29405 Salary $126,500 - $148,500 (wider range available depending on experience and location) Role Description The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on Medicare, keeping them happy, healthy, and out of the hospital. NP’s work independently and collaborate with physicians on a team that includes medical assistants, scribes, nurses, and care managers. Core Responsibilities Provide evidence‑based primary care and conduct office visits for routine and acute needs. Administer annual wellness visits and health risk assessments, documenting holistically. Coordinate care with other providers, specialists, and agencies. Lead population health activities, ensuring appropriate screening and preventive services. Support phone triage, outreach, and patient education. Participate in promotional activities and perform home visits as needed. Report to the Center Medical Director and collaborate with operational leadership. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family NP, Adult‑Gerontology, Primary Care NP, Adult NP, or Gerontological NP. Active, non‑probationary state NP license and active DEA license. US work authorization. Preferred Qualifications Experience in primary care, internal medicine, geriatrics, or family medicine. Knowledge of HCC, ICD‑10 coding, and health risk assessments. Team‑oriented mindset and experience with cross‑functional collaboration. Desire to participate in an innovative, evidence‑based population health model. Bilingual proficiency in applicable languages. Pay Range The typical pay range for this role is $78,331 – $168,714, adjusted for experience, education, and geography.
