Nurse Case Manager
Constructive Partnerships Unlimited (CP Unlimited) was formerly known as Cerebral Palsy Associations of NYS, Metro Division. Our new name better reflects our wide array of services and programs and the diversity of people with disabilities that our agency supports. CP Unlimited is one of the New York City's largest and most comprehensive community service organizations serving people with developmental disabilities and their families.

We are currently seeking a Nurse Case Manager for our Manhattan residences.
Responsibilities include:
- Coordinate the delivery of health care services for adults with disabilities residing in residential programs.
- Perform nursing assessments and diagnoses
- Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
- Provides guidance and training to direct care staff to ensure proper delivery of health care services.
Requirements include:
- NYS RN License
- Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
- Excellent oral and written communication skills.
- Ability to travel to and access multiple locations throughout the metropolitan area.
- Able to work flexible hours and/or days.
