Physical Therapist FT

Haven Home Health & Hospice is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team. This position offers the opportunity to make a meaningful difference in the lives of patients by helping them improve mobility, regain independence, manage pain, and achieve their highest level of function in the comfort of their own homes.As a Physical Therapist with Haven, you will work collaboratively with physicians, nurses, therapists, and caregivers to develop and implement individualized treatment plans that support each patient's unique goals and needs.ResponsibilitiesEvaluate patients and develop individualized physical therapy treatment plans.Administer therapeutic exercises and treatment interventions to improve mobility, strength, balance, and function.Assess patient progress and modify treatment plans as appropriate.Educate patients and caregivers on exercises, safety techniques, and ongoing care needs.Recommend and instruct patients in the use of assistive devices and equipment.Collaborate with physicians and interdisciplinary team members to coordinate patient care.Complete accurate and timely clinical documentation.Supervise and collaborate with Physical Therapist Assistants (PTAs) as assigned.Participate in team meetings, case conferences, and ongoing education opportunities.QualificationsCurrent Oklahoma Physical Therapist (PT) license required.Graduate of an accredited Physical Therapy program.Current CPR certification.Valid driver's license, reliable transportation, and current automobile insurance.Ability to travel throughout the service area.Excellent communication and interpersonal skills.Home health experience preferred, but not required.BenefitsCompetitive CompensationMileage ReimbursementFlexible SchedulingPaid Time OffMedical, Dental, and Vision InsuranceSupportive Team EnvironmentOngoing Training and Professional DevelopmentOpportunity to Make a Meaningful Impact in Patients' LivesWhy Haven?At Haven Home Health & Hospice, we believe exceptional patient care starts with exceptional team members.
